Basic principles of LTCT
In the case that it is impossible to lock, hang a "dangerous operation prohibited" label, and arrange a special person to monitor.
The lock or Lockout tag shall be released by or under my sight. If I am not present, "abnormal unlock" should be implemented.
If a shift operation is required, the shift replacement should re-execute the Lockout Tagout procedure and keep Lockout Tagout at all times during the work.
The duties of a LTCT
Preparation of isolation plan before operation;
Conduct isolation and release tests to ensure operational requirements are met;
Lock all isolation points and put "Dangerous Do not operate" label;
After the completion of the operation, confirm that the facility meets the operation requirements, and unlock it after the signature of all operation personnel.
